For your convenience and to make sure your payment is credited properly, please follow these instructions:

1. Print the Payment Enclosed Form.
   Get Adobe Reader
2. Complete the form and enclose it with your payment.
3. Make your check, money order or cashier’s check payable to Bakalar & Associates. Your cancelled check will be your receipt.
4. Do not send cash through the mail.
